Um certificado impresso emoldurado por uma figura esfolada de pé à esquerda e um esqueleto à direita, ambos sobre pedestais de pedra em uma paisagem. As duas figuras seguram um pequeno medalhão circular com uma figura de manto ao lado de um esqueleto, e o texto escrito ocupa o centro.
